Key Responsibilities

• Lead and optimize care coordination workflows and clinical operations across care programs.
• Oversee patient intake, registration, and provider scheduling for home visits and telehealth consultations.
• Directly manage and mentor Care Coordination Team Leads and operational staff.
• Ensure timely completion of service recommendations and follow-up clinical visits.
• Collaborate closely with the Clinical Operations Lead to maintain high standards of patient care quality.
• Report operational metrics, bottlenecks, and program health directly to executive leadership.
• Monitor compliance with healthcare standards, regulatory guidelines, and internal protocols.
• Drive performance monitoring and continuous process improvement across remote patient monitoring operations.
• Coordinate with strategic partners and external healthcare providers to streamline patient transitions.
• Analyze operational datasets to identify trends, forecast resource requirements, and enhance efficiency.
